Summary

The Registered Nurse II Permanent Supportive Housing Home Health performs a variety of duties to provide quality patient care to the sick or injured or others requiring healthcare services at contracted Permanent Supportive Housing Sites. The scope of services includes care for men and women in Permanent Supportive Housing. This position reports to a Healthcare for the Homeless – Houston (HHH) Family Nurse Practitioner.

Job Duties

  • Adheres to the scope of practice for Registered Professional Nurse per State regulatory guidelines; assume responsibility for maintaining compliance according to Federal, State and Local regulations and agency policies and procedures.
  • Demonstrates appropriate judgment & skills to make independent clinical decisions in routine patient care matters and triage of health complaints in conjunction with supervising clinician.
  • Facilitates telemedicine appointments for permanent supportive housing clients by utilizing provided equipment (MedPod with cameras, scopes, vitals equipment, and EKG). 
  • Performs phlebotomy for adult patients.
  • Demonstrates knowledge of therapeutic action, side effects and interaction of medications – administer medications to permanent supportive housing residents per doctor’s orders.
  • Reviews results of laboratory tests and other diagnostic exams for assistance in navigation of care
  • Evaluates or assists in evaluation of appropriateness of Plan of Care and revise on an ongoing basis with communication to other disciplines and providers
  • Coordination of clinical care (appointments, procedures, medical records, health promotion) 
  • Develops and facilitate health promotion activities, classes, workshops, etc. 
  • Remains flexible and responsive when changes occur in patient activity and workload.
  • Fosters an environment for open communication and collaborative practice.
  • Plans, teaches, supervises and counsels regarding physical care measures to promote improvement/recovery, symptom/complication management, expected disease progression, social/emotional care and community resources.
  • Acts as a comprehensive care coordinator for patients assigned to his / her care, assessing the patient’s needs accordingly and facilitate communication amongst other clinicians providing care.
  • Performs comprehensive assessments of physical, emotional, psychosocial, and environmental needs of the patient including outcome and assessment information.
  • Completes and submits all required documentation within established Agency guidelines; maintains patient record in the HHH electronic health record. 
  • Interprets the appropriate information needed to identify each patient’s requirements relative to his or her specific needs.
  • Develops and implements a realistic individualized Plan of Care, as appropriate.
  • Compares assessment data to baseline assessment to monitor patient’s progress. 
  • Recognizes alterations in function, including life-threatening situations; intervene and document appropriately.
  • Participates in development of patient goals and discharge plans as well as make revisions to these. 
  • based on the changes in patient status. 
  • Initiates patient education based on identified learning needs of the patient and/or those providing care; provide wellness training in individual and group settings.
  • Performs other tasks or special assignments as necessary to maintain proper operation and patient care.


 

Minimum Qualifications

  • Associate's Degree or Bachelor's Degree in Nursing.
  • Two years of relevant experience as a licensed Registered Nurse (RN). 
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
  • Basic Life Support (BLS) Certification.

Preferred Qualifications

  • Two years clinical experience, preferably with a medically underserved population.
