Case Manager- IMPACT Psychiatry



OU IMPACT seeks to provide intensive treatment to persons with chronic mental illnesses. Our primary objective is to improve overall wellness for those individuals that need higher levels of care, by utilizing a best practice approach that incorporates clinical, medical and rehabilitation services through an integrated multidisciplinary team.   Our target population will be those individuals that are diagnosed with schizophrenia, schizoaffective disorder, recurrent major depression with psychotic features and bipolar disorder. The core philosophy of OU IMPACT is based on harm reduction and strength building. OU IMPACT provides a best practice approach to treatment and prevention and does so through engagement, encouragement and positive reinforcement.



Position Information: Responsible for individual rehabilitation, case management, assessment/reassessment, service planning, emergency response, monitoring and discharge of consumers.



Essential Duties:




  • Provides support to cope with chronic or acute illnesses or conditions.

  • Advises care givers, provides patient education and counseling, and makes referrals for other services.

  • May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

  • Provides individual rehabilitation to include teaching, educating, modeling etc, to build skills and increase quality of life. 

  • Collects in-depth information about a client's situation and functioning through client interview and other relevant sources to identify individual needs.

  • May develop a comprehensive treatment plan that will address identified needs. 

  • Monitors, and evaluates treatment plans and progress. Facilitates interdisciplinary approaches.

  • May develop and implement action-oriented and time specific treatment plans by determining and documenting specific objectives, goals, and actions designed to meet the client's health and human service needs identified through the assessment process.

  • Organizes, secures, integrates, modifies, and documents the resources necessary to accomplish the goals set forth in the treatment plan.

  • Gathers information and documentation from all relevant sources regarding the treatment plan's activities and/or services to determine the plan's effectiveness in reaching desired outcomes and goals.

  • Measures and analyzes the interventions to determine the outcomes of case management involvement, e.g. clinical, financial, variance, quality/quality of life, client satisfaction.

  • Performs other duties as needed to successfully fulfill the function of the position.

  • Will provide crisis intervention services.

Working Conditions:




  • Physical: Walking, climbing stairs, getting in and out of vehicle. Speaking and listening. 

  • Environmental: Community environment, weather, some office Environment.



Experience:




  • Required: 12 months experience in home or community-based services, case management, or a related healthcare environment.



Education:




  • Required: Bachelor's Degree in Social Work, Psychology, Sociology, or other Health or Human Services related field OR accepted degree for Case Manager II accepted by The Oklahoma Department Of Mental Health and Substance Abuse Services.



Knowledge, Skills & Abilities:




  • Knowledge of federal and state laws particularly patient confidentiality.

  • Knowledge of case management fundamentals and the technical/procedural aspects of case management.

  • Must have a high level of organization skills.

  • Ability to resolve issues, applying judgment, critical thinking, and problem solving skills to determine a course of action.

  • Ability to exercise discretion when making decisions and seeks management’s advice to determine proper course of action while following established standards.

  • Ability to handle stressful situations, different situations or circumstances, and complete projects in a timely manner.

  • Ability to keep accurate and detailed records.
