Job Description

Summary

We are seeking a compassionate and organized Patient Navigator to join our team for a new project focused on improving healthcare transitions for adolescents with primary immunodeficiency (PI). The navigator will meet with patients and families regularly to provide education, assist with health literacy assessments, guide them through obtaining health insurance, and offer support as they transfer from pediatric to adult medical providers. This part-time role (0.4 FTE on Mondays and Wednesdays) is an excellent opportunity to make a difference for an underserved patient population by coordinating care and interventions to facilitate successful transitions. This role will serve 60+ PI patients and require collaboration with various departments and providers. Strong communication skills and compassion are essential to provide vulnerable families with needed support during healthcare transitions.

Job Duties

  • Meets with patients ages 12-26 with primary immunodeficiency (PI) and their families to provide education and support around transitioning from pediatric to adult healthcare. Build rapport through regular in-person meetings and follow-up contacts.
  • Administers health literacy assessments such as the Transition Readiness Assessment and Transition Planning Tool to 60+ adolescent patients. Track progress longitudinally.
  • Ensures patients fully understand their insurance options.
  • Guides patients and families through obtaining health insurance coverage including Medicaid, CHIP, Medicare, Affordable Care Act plans, or employer-based plans. Assist with paperwork, documentation, and applications.
  • Serves as the main point of contact for patients and families throughout the transition process.
  • Documents all interactions and progress clearly in patient electronic medical records. Maintain organized records.
  • Works closely with project lead/PI and other care providers to identify needs, provide input on interventions, and improve transition program over time.
  • Participates in team meetings and assist with evaluating program outcomes. Help prepare summaries and reports.
  • Completes other duties as assigned by the PI.
  • Documents all activities, service plans, and results.
  • Maintains confidentiality per HIPAA guidelines in regards to patient information.
  • Represents the Division, and the programs at community events.

Minimum Qualifications

  • High School diploma or GED.
  • Four years of relevant experience.

Preferred Qualifications

  • Bachelor's degree in social work, public health, nursing, or related field.
  • Two plus years experience working directly with adolescents and young adults.
  • Background in pediatric/adult healthcare, chronic disease management, or disability services.
  • Experience with healthcare transition processes and models (GotTransition, CMMS, etc.).
