Position Summary:

The RN Community (Surgical Oncology) Care Coordinator works with an interdisciplinary team to serve as a patient advocate and educator for patients with cancer. The Care Manager determines the needs of the cancer patient and caregivers and facilitates access to appropriate City of Hope and community resources in a timely manner. The RN Community Care Coordinator will educate patients and caregivers about the disease process, treatment options through consultative planning and appointment coordination with physicians and other ancillary services. Collaborates in the development, coordination and implementation of support functions related to cancer including but not limited to patient and caregiver education, community outreach, advanced care planning, clinical trials, health literacy, support groups and classes. Identifies potential/actual barriers to care and works with patient and caregiver(s) to eliminate barriers. Serves as a professional resource in the cancer care continuum and disease process for solid tumor malignancies.

Essential Functions:

Nursing Process and Practice

  1. Uses nursing process to assess, plan, implement and evaluate patient care.
  • Consistently completes initial assessment within the timeframe specified for the patient care area
  • Demonstrates clinical knowledge base and provides care that meets professional standards of practice.
  • Interprets assessment data
  • Identifies abnormal assessment/problems and recognizes abnormal diagnostic findings
  • Implements nursing interventions based on plan of care developed from assessment, data, and diagnostic findings
  • Evaluates effectiveness of nursing interventions
  • Provides follow-up regarding clinical issues
  • Assesses and reassesses patients at regular intervals based on data from history, signs and symptoms and interprets diagnostic data
  • Anticipates potential problems based on data from history signs and symptoms and interpretation of diagnostic data
  1. Assesses patients appropriately in relation to their disease process, acuity of illness and psychosocial needs.
  • Takes actions in response to change in patient’s condition
  • Uses assessment data to develop individualized plan of care for both physical and psychosocial needs
  • Interprets complex diagnostic data and accesses current literature to understand disease entities
  • Serves as a resource to colleagues to help integrate assessment, history, and diagnostic findings into the plan of care
  1. Delivers patient care according to Policy.
  • Performs nursing care for a full appropriate assignment
  • Delegates tasks as required by changes in patient acuity
  • Utilizes resources appropriately (i.e., CNS, wound care team, LCSW, dietitian)
  • Consistently anticipates and helps resolve potential problems with patient plan of care

Education / Patient, Family, Community and Self

  1. RNIVs demonstrate assessment of patient and family health literacy to determine patient’s/family’s ability to comprehend and understand education.
  • Proficient at facilitating comprehension, minimizing/eliminating barriers and providing patient – centered education.
  • Employ educational strategies such as teach back, inclusion of designated caregivers and validating education of individual patients
  1. Provide education on a macro level.
  • Contribute to the education of our communities about health, wellness, prevention, and continuum of care issues relevant to the communities.
  • Leader in the review of current literature, attendance at educational programs, workshops, and in-services, with dissemination of information back to peers.
  1. Committed to lifelong learning, including on-going pursuit of education, both formal and informal.
  • Strive to exceed the minimum expected level of education for the professional registered nurse (the bachelor’s degree), pursue professional certification and promote participation in continuing education offerings both internal and external.

Basic education, experience and skills required for consideration:

  • Bachelor’s Degree in Nursing required
  • Master’s degree in Nursing acceptable if Bachelor’s degree is not in Nursing.
  • Minimum two years oncology nursing experience

Required Certification/Licensure:

  • Current California RN license
  • Current National certification, must be maintained in order to retain RN IV status
  • Current Basic Life Support (BLS) CPR Card through American Heart Association
  • ONS Fundamentals of Chemotherapy Immunotherapy Administration (or obtain within 6 months of hire)
  • OCN certification (or to be obtained within 1 year of hire)

Pay Rate: $54.43 - $81.64 / hour
